Do I need to schedule an appointment for immunizations?

Am I or my child up to date on immunizations?
Call 812-752-8455 and we will check your records in the State of Indiana CHIRP system, Children and Hoosier Immunizations Registry Program. If your child has had immunizations in another state, you will need to contact the place they received the immunizations and get a record before we can tell you what is needed.
What do I need to get a birth certificate?
In the State of Indiana, you have to go to the county where you were born to obtain a birth certificate. If you were born in Scott County, you will need to fill out an application and have a current ID. The cost is $10, and we only accept cash or checks on local banks. As long as everything is correct in the system, you will be able to obtain a certificate the same day you apply.
When can I get a TB Test?
TB tests are given on Mondays and Tuesdays between the hours of 8:30 a.m. and 11:00 a.m., and 1:00 p.m. and 3:30 p.m. at the Scottsburg location. The cost is $10, and we only accept cash or checks on local banks. The tests are then read in 48-72 hours after they are given between the hours of 8:30 a.m. and 3:30 p.m.
Where is my septic tank located?
Call the Environmentalist, Tim Brunner, to see if there is a drawing on file. Usually it is located 10 ft. away from the main floor bathroom.
